Health & Intake Information
Health information you provide on intake forms is used solely to inform session planning and to ensure your safety during bodywork. It is not shared with third parties except as required by law.
Health records are retained for a minimum of seven years following your last session, consistent with Arizona general records retention guidelines for professional services.

This website is hosted on Squarespace, which uses cookies to operate the site, analyze traffic, and maintain functionality. We do not use advertising cookies or behavioral tracking beyond what Squarespace provides by default.

We rely on GlossGenius and Squarespace — both established platforms with industry-standard security practices — to store and transmit your data securely. Fascia Rebalanced does not maintain local databases or servers. Payment card data is never stored directly by this practice.
Physical session notes and intake forms, if maintained in paper form, are stored in a locked location accessible only to the practitioner.
